Short_breaks_16We understand that when a young person reaches adulthood, a move away from the family home or leaving a residential college can be a very daunting and intimidating experience and process.

Help is at hand. Our team is available to offer the young person, their family, carers, commissioner, advice, support and information on a range of issues - from housing provision, day time activities, employment and care, to individual and personal budgets and benefits - to help them choose the right support and care package to meet their current and future needs.

They may be looking for us to provide supported living for a few hours a week, or 24/7, either within their family home or as a tenant in rental accommodation. Whatever their needs, we can tailor our services to meet their requirements.

Experience has shown that by undertaking transition planning at an early stage, even as young as 14 years old, and involving each person and their circle of support every step of the way, we can help alleviate fears and anxieties, and make the transition from children’s care services to adult care a much easier one.
