To enable effective empowerment, decision making and to maintain dignity, we provide you with the tools and channels to communicate effectively with everyone around you.
Working together and with input from your network of health professionals, a health action plan is developed which links into the outcomes identified within your support plan, risk assessments, behavioural management plan (where relevant) and communication passport.
We support you:
- by working in partnership with a wide range of health professionals to support your health needs and general well being
- to help manage any medication you are taking
- with social issues or practical problems relating to your complex health needs
- to utilise individual and group therapies, including alternative therapies
- with practical aids and support to cope at home and in the community (including using assistive technology)
We ensure that you are registered with a G.P. and a dentist and support you to access other services as appropriate, such as occupational therapy, physiotherapy, behavioural teams, community nursing, chiropody, ophthalmologist and dietetics. We also support and encourage you to participate in living a healthy lifestyle through healthy eating, exercise and health improvement programmes.
